Diabetic retinopathy is a leading cause of blindness in Singapore.
Primary Care Coordinators from Class PCN will help to coordinate
periodic DRP screenings to enable early detection. Our Nurse
Counsellor will take a photo of the back of your eye and your GP
will follow up if there is a problem. Early detection and monitoring
can reduce the risk of developing blindness. Eye screening services
to the patients in our network drastically reduce your waiting time.
Spirometry is a pulmonary function test.
It measures lung function, specifically the amount and/or speed of
air that can be inhaled and exhaled. Spirometry is helpful in assessing
breathing patterns that identify conditions such as asthma, COPD or
other conditions that affect breathing.
Our Nurse Counsellors will collaborate closely with you and your doctor in developing a personalized chronic disease management care plan, taking into consideration your concerns and needs.
Education topics include:
- Self-management (home monitoring of blood glucose and management of hyperglycaemia and hypoglycaemia)
- Techniques for insulin injection
- Education on techniques for insulin injection and correct use of inhaler for asthma/COPD inhaler
- Techniques on correct use of inhalers for asthma/COPD conditions
- Lifestyle and dietary modifications
- Risk factor assessment
Our Care Coordinators will work closely with you to coordinate your appointment for the ancillary services determined by your doctor as well as follow-up care. You will also receive appointment reminders to ensure you receive the personalized care for your chronic disease condition(s).
